Post Hospital Discharge Review

Post hospital discharge, patients are are increased risk of deterioration and reversion into their condition, therefore we offer daily check in services for the days following discharge, ensuring patients any deterioration is rapidly detected, and reintegration into their home is made easy.

Mental Health Review

Mental health remains the most common health concern in those living with disability, therefore our trained staff will ensure that all patients receive mh monitoring and routine catchups with companionship and other treatment being provided when needed.

Proactive Health Assessments

In depth assessments completed by registered paramedics with clinical oversight by GP which ensures that any issues detected are immediately recognised with a treatment plan created.

Emergency Service Liaising

In the event of an Emergency Ambulance being called for a patient, we will provide a handover with all pertinent patient history, recent trends and vital information on your behalf, acting as your site medical team so you dont have to.
